Every OR is different from another. Surgeons, anesthesiologists, nursing, administration, decades of habits, relationships and processes contribute to a very complicated puzzle. With the assessment offering, Brighthospital puts together “what works” with a deep understanding of the individual ORs and their issues. It combines an objective, clear and benchmarked picture of the performance the ORs with a path of improvement that takes into account the barriers that face them. Brightanalytics Surgical Analytics Service:
At the core of Brighthospital’s surgical services assessment is the Brighthospital Surgical Analytics Service. This service has been developed from ground up to be the most comprehensive analytics offering for surgical services. The service aims to provide a complete and accurate picture of the performance of a hospital or ambulatory surgical services ORs, both internally in and how the ORs interact with other areas in a hospital such as inpatient services and ER. The components of the analysis include review of key operational indicators including:
- Surgeon productivity
- Utilization analysis of ORs, blocks, prime and non-prime time,
- Turnover time analysis - single room, multiple room, surgeon perception, variation by time of day,
- Staffing / Rooms running analysis; shift structures, staff distribution,
- First case start, delay analysis, delays that shift into non-prime time,
- Schedule predictability analysis: case duration estimation accuracy, case change index
- Case smoothing - impact of surgical schedule on inpatient utilization
- Length of stay analysis, both perioperative and inpatient review.
In addition to these operational indicators, the service provides the ability to perform a full financial analysis of the ORs. The initial review depends on the available data that the site can provide. In a full subscription, Brighthospital works with the sites to expand the accuracy of this analysis by improving both quantity and accuracy of the data that is being captured.
One of the key requirements of an effective analysis is understanding the underlying data fully. During assessment begins with an inventory and definition review of the available data in the perioperative environment and other sources within the hospital. This provides the foundation that is required for believable and actionable analysis. This phase is followed with the creation of the analysis environment and creation of the initial snapshot. This initial snapshot is reviewed with the leadership of the ORs and provides a template for the onsite component of the assessment. Upon completion of the assessment, the service will be available for a duration to the OR leadership for analysis and can be refreshed on regular intervals as a subscription service.
ON-SITE Review:
On-site review tries to combine the performance of the ORs as documented by Brightanalytics with in-person review of the processes and issues. These include but are not limited to:
- Operational reviews: Scheduling, pre-admission testing, case preparation, turnover time
- Financial reviews: Charging, charge master, financial clearance processes
- Policy review: Scheduling rules / bylaws / block management processes and rules
- Information systems usage
These reviews are followed with a set of interviews of key physicians and senior leadership. The physician interviews focus on specific issues that the physicians are either facing or perceiving with the ORs. Depending on the need, these interviews can be supplemented with customized questionnaires or online surveys. The purpose of the senior leadership interviews is to understand the strategic direction of the hospital and the steps the OR will need to take to reach the strategic objectives or to clarify the information the leadership needs to set an informed strategic direction.
The final step of the assessment is communication. A successful assessment needs to end with a consensus of opinions on the direction the ORs need to take. Therefore, the results of the assessment is reviewed initially with the OR leadership and then with as many parties and times as necessary, including senior leadership, physicians, nursing and board of directors as needed. Of course, a comprehensive report, all the analysis created and a limited time access to live Brightanalytics environment is the standard leave-behind for all engagements.
